Common questions
How often will I need visits?
It depends on how well controlled your condition is. For diabetes, many patients come in every 3 to 6 months.
Should I check my numbers at home?
If your doctor recommends it, yes. Bring your home blood pressure or blood sugar readings to each visit; they help us adjust your plan.
Do you work with my specialists?
Yes. We coordinate with cardiologists, endocrinologists and other specialists so your care stays in one plan.
When should I book a hospital follow-up?
Call as soon as you're discharged. A visit within the first week or two helps catch problems early.
